INSURANCE & PATIENT CO-PAYMENTS

If you have insurance, we’re happy to help you understand your coverage and file claims on your behalf. To best assist you, please send us your dental and/or medical insurance details along with your new patient paperwork at least two business days before your appointment. This gives us time to review your benefits and provide a clear estimate of what your insurance may cover and what you’ll be responsible for at the time of service. While we’ll do everything we can to help, please remember that your insurance is an agreement between you and your provider, and you are ultimately responsible for your care.

In an effort to reduce the cost to you, the patient, while maintaining the highest level of professional care, we have established the financial policy as follows:

  • Payment of any account balance is due in full within 20 days of statement date
  • We gladly accept Visa, Discover, American Express or MasterCard payments
  • Payment plans are available through CareCredit

APPOINTMENTS & CONSULTATIONS

We’re happy to offer second-opinion consultations to help you feel confident in your treatment decisions. All patients seeking a second opinion must first complete an exam with our doctor before a diagnosis can be given. To ensure a thorough evaluation, please have all X-rays and chart notes from your previous office sent to us before your visit, keeping in mind that transfers may take 7–14 business days. Be sure also to provide any dental or medical insurance information ahead of time so we can prepare a detailed treatment estimate after your consultation.
